very wide ranging questionaire - I'm going to have a problem with some of the questions though - I've had not contact with my parents& sister since 1976 & have no idea if they are alive or dead - the one thing that does suprise me is that although they ask about drug & alcohol use/abuse they don't seem to test for it - not that I'm worried about that being teetotal - I'll alos have to look back & find remember when I broke my ankle as that was the only time I was in hospital
