We finally managed to make appointments in Norwich for the third week in January, so fingers crossed our ITA can be sent by Feb 1st. If anyone is looking for an idea of cost, our total bill for 2 adults and 3 kids (aged 6 and under) is ?380, plus ?100 for x-rays.
